I am a social worker in a nursing home in the New Orleans area and trying to assist a resident who desires to transfer to a facility in Lorain Ohio to be closer to family. The resident is a 40 year old male who is vent dependent due to a spinal cord injury resulting from a fall at work. Resdient is alert and oriented. Good family support but lives out of state and desires for resident to be closer.
