Home Care Certified Nursing Assistant (CNA), Program of All-Inclusive Care for the Elderly (PACE) Role: Home Care Certified Nursing Assistant (CNA), PACE Center of Northern Virginia Location: Washington DC metro area/Northern Virginia (Alexandria) Status: Full-time Mission: To allow frail elderly people to age in the location of their choosing and continue to lead connected, meaningful lives Role in Brief: This Home Care Certified Nursing Assistant (CNA) role offers a unique opportunity to help our frail elderly population to continue to lead connected, meaningful lives. Edenbridge Health, in partnership with ConcertoHealth, is developing a PACE program for the Washington DC metro area, located in Northern Virginia. We are looking for an experienced Home Care Certified Nursing Assistant who will be responsible for providing and assisting participants with activities of daily living at home. Main Responsibilities: • Assists participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed. • Monitors health status and provides a safe environment for participants. • Follows home care schedule for daily/weekly assignments • Participates in participant’s care planning through cooperation with interdisciplinary teams. • Promotes professional working relationships with both internal and external customers. • Reports to Home Care Coordinator • May rotate into day center services as a CNA as applicable and deemed necessary by the Clinic Manager or Home Care Coordinator. • May rotate into the clinic as a CNA as deemed necessary by the Clinical Nurse Manager. • Staff may be required to utilize their personal vehicle to meet participants at contracted provider’s offices based on staffing needs. • Participates in Quality Improvement programs. • Consistently cooperates and supports organization in problem solving issues. • Participates in continuing education classes and any required staff and training meetings. • Be flexible in schedule of hours worked. • Performs other duties as assigned. Qualifications: • Must possess, as a minimum, a GED or high school diploma. • Must be an active certified nurse assistant in the state of Virginia. • Minimum of one (1) years of documented experience working with a frail or elderly population preferred. • Experience in a long-term care facility or with a community-based geriatric program preferred. • Knowledge of the principles and techniques involved in relating to frail elderly persons. • Skill in communicating with participants, their families, and other staff members; ability to establish a helping, trusting relationship with participants. • Ability to follow through with instructions and directions provided by supervisor, RN, QAPI coordinator, primary care physician, and/or medical director.
