Elder male urine incontinent, needs help with bathing, changing, dressing, feeding, and walking. impaired, as balance issues and is slow to comprehend at times. Have had leg injury and back problems. Able to do some things on his own, but needing more help with dressing, dp changes, and some feeding assistance. than in the past. Friendly, gentle, but confused and very sad at times. In need of physical and emotional support. taking meds, but needs reminders. Also, has trembling hands at times. Uses cane as needed, vision blurred at times. Overweight, #2 diabetic, weak leg, needs assistance with daily cares. Lives alone.
