RESPONSIBILITIES: Responsible for the planning, implementation, and delivery of direct clinical services provided by nurses to hospice patients and their families. Provides guidance to the Interdisciplinary Team, to assure the effective use of resources in the provision of care. REPORTS TO: Chief of Patient Care Services FUNCTIONS: Oversee the planning, coordination and delivery of the Plan of Care for the patients assigned to nurses under your supervision. Oversee the clinical supervision of assigned nursing personnel and conduct written performance evaluations. Provide information for inclusion in written performance evaluations for all patient care staff in cooperation with appropriate members of the clinical management team. Participate in and provide leadership for the team during regular Interdisciplinary Team meetings. Supervise the development, review and revision of the patient/family plan of care, for patients assigned to nurses under your supervision Provide leadership and counsel to assigned nursing personnel Participate in the Administrative after-hours schedule. Documentation is entered into the Cerner Electronic Medical Record System. Documentation is completed within agency guidelines and to meet regulatory requirements. Collaborate with the Chief of Quality & Compliance in assuring medical records’ accuracy, compliance and timely completion Perform scheduling, supervision and assist in the ongoing education of Home Health Aides. Participate in nursing staff selection and orientation of new personnel and student interns to applicable patient care responsibilities and policies Identify need for staff education. Assist agency in the development and/or updating of the hospice nursing training course segment Participate in the agency’s Quality Assurance / Performance Improvement (QAPI). Perform specific assignments as directed by the Chief of Patient Services and the Chief Executive Officer. Participate in Hope Hospice’s orientation and in-service training programs for professional staff. Participate in agency and community programs as requested to promote professional growth and understanding of hospice care, as well as infection control information. Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies that aid in determining policies. QUALIFICATIONS: Possess a current license to practice as a registered professional nurse in the State of California Preferably a B.S. degree in nursing with a Public Health Nurse (P.H.N.) certificate Minimum of three years varied work experience as a professional nurse with preferably two of the three years within an acute, oncology, home health or hospice setting Two years’ nursing experience within the last five years in a home health agency, primary care clinic, or health facility Minimum one year previous supervisory experience in a health care setting strongly preferred Demonstrated ability to manage all aspects of terminal care with expertise in pain and symptom management and hospice nursing Demonstrated skill in administrative performance, communication, supervision of staff, documentation requirements, service coordination Must have clinical knowledge and critical thinking ability to provide coordination of patient care consistent with standards and regulations Excellent organizational and management skills to handle complex patient care issues Possess a valid California driver’s license, valid auto insurance, and an automobile for field use Requires manual dexterity sufficient to provide hands-on care to the patient under variable conditions and settings Must be able to operate standard office machines such as fax, telephone, etc. Must have strong computer skills or willingness to achieve necessary skills Must be free of communicable diseases Must be able to communicate clearly in English verbally and in writing
