Sir/Madam: I would like to apply as a live in caregiver here in Canada. I earned the Bachelor of Science in Nursing and excelled as Cum Laude in the Philippines. I am a licensed Registered Filipino Nurse. I believe that my experience in various hospitals and health offices enhances me to perform efficiently and effectively, and impressed upon me are the importance of a deep sense of responsibility, commitment and teamwork. I also attended seminars and training to update my knowledge and enhance my skills as a nurse. I worked in areas such as general ward and emergency room as a staff nurse here and abroad in which I work well with patients of any age. I am a hardworking, kind, patient, passionate and caring person. • Change linens and make beds • Prepare and plan meals (including clean-up) • Help with walking and light exercise • Plan ahead for future meals, check expiration dates on food • Do laundry (may include light ironing) • Light housekeeping duties (such as vacuuming and dusting) • Assist with dressing, grooming and bathing • Run small errands (pick up prescriptions, go to the bank, etc.) • Remind when to take medications • Engage in mental and physical exercises • Escort to appointments (doctor, physical therapy, hair salon, etc.) • Organize mail and maintain a schedule/calendar • Escort to religious services • Engage in regular activities (talking, games, etc.) • Provide companionship • Record daily care notes • Make note of any significant changes in the client to the agency/service • Gives direct nursing care to general and elderly patients. • Identifies nursing needs of child and elderly patients. • Thorough assessment of the patient to determine appropriate nursing implementation. • Plans, implements and evaluates nursing care. • Admits patients. • Performs nursing activities such as bathing and oral hygiene. • Assists patients in moving, maintaining proper body alignment and utilizing good body mechanics. • Takes and records vital signs. • Prepares and gives oral and IV medications. • Observes and responds to patients emotional and spiritual needs. • Acts as liaison between patients and hospital personnel. • Interprets to the patients and his family their role in promoting successfully therapy and rehabilitation. • Collaboration with the health care team in planning and evaluation of care provided. • Prepares patients for and assist physicians with diagnostic and therapeutic procedures. • Performs irrigation’s catheterization and gives enemas. • Prepares/implements care plans. • Gives due medications.
