Hello, I’m addition to offering traditional elder, child, pet, and home care, I offer a patient advocacy service. Let me help! Rates vary-job dependent. I would like to provide a unique service for your elder care needs. I will provide in home care one day a week for a time period as needed to establish a relationship and evaluate the needs of the elder. I can work with other caregivers as needed. I will organize medical records, medication schedules, physician information and recommendations. I can educate the elder, other caregivers, and family members regarding current diagnoses, medications, etc. I will assess the environment and make safety recommendations (fall risks, potential medication issues, etc.) as needed. If necessary and desired, I can serve as a liaison with the physicians to ensure seamless care between providers. I am willing to accompany elders at physician appointments to ensure full understanding of recommendations and documentation of medication changes, etc. Elder care can be very complex and stressful for the family, as well as for the elder. I can provide peace of mind and patient advocacy to ensure organized comprehensive care for your loved one.
