Elder Care Job in Modesto, California

I am looking for someone to provide transportation for my mother to physical therapy on Monday and Wednesday. She moved to assisted living in November 2015. She has some dementia and uses a walker. She is very friendly and likes to talk. She is not the type of person that adapts to change easily.
