Introduction We are Radio CW, a non-profit organization based in Gaithersburg, Maryland since 2009. After 6 years in the community, we intend to have more of an impact in the community than we have had in the past. We are aiming to work for our community by establishing an ethical assisted living and in home care. Since our inception, Radio CW’s has maintained a fundamental spirit of teamwork. Teamwork is a very important value as we will be looking after members of our community, and a strong team collaboration is needed to insure the safety and wellbeing of our community members that are put in our care. There’s no place like home, and we mean real homes, not nursing homes. There is a great need to house individuals with disabilities, allowing them to live and blend in the community with real support instead of being locked away in nursing homes, and other institutions. The determination that a disabled applicant can safely return to his or her home is a decision that should be made in conjunction with the applicant’s physician or other responsible health care practitioner, and where possible, the applicant’s responsible family members. The home care we are proposing is a safe and ethical community centered service. Our objective is an affordable home that delays institutionalization. Our purpose is to improve staff stability, to increase person centered care planning, to prevent and to manage infection safely. We train our staff. screening, do the background check and TB test before send them to the workplace Program Purpose, Goal and Objectives GOAL: To provide safe and ethical care, that enables seniors to remain in their own home with a high quality of care for as long as possible though the provision of appropriate home and community-based services, including support for family caregivers. Offering a compassionate nature and expertise in providing personal care to senior residents. OBJECTIVES: Provide affordable home and community-based care services and living options to prevent or delay institutionalization. • Demonstrated expertise in assisting patients with bathing, grooming, toileting and ambulation • Committed to providing both emotional and social support to residents and their families • Focused on providing watchful supervision and observing patients’ conditions with a “hawk’s eye” • Track record of following prescribed healthcare and medication plans, with the aim of ensuring residents’ physical and mental wellbeing • Excellent skills in charting out appropriate recreational activities for residents and encouraging them to take part in them • Particularly effective in communicating with patients and families regarding specified care and educating them about home care regimes • Proficient in preparing food according to each resident’s specified calorie intake and ensuring that they partake their meals on time • Documented success in encouraging residents’ independence and dignity through a series of counseling sessions • Well-versed in maintaining a safe environment by ensuring that surroundings are free of object that may cause accidents • Special talent for maintaining a professional, yet compassionate image towards residents, visitors, families and coworkers • Knowledge of policies and procedures that govern the running of assisted living facilities • Qualified to handle emergency situations using strong knowledge of applying First Aid and CPR PURPOSE: • Improving staff stability: Shaloom Healthcare Staffing believes that addressing staff stability is a cornerstone to all the other work. We aim to provide a safe and ethical environment for our facility, thus allowing a lower turnover of staff, and creating a long term familiar setting. If you’re having high turnover, it’s hard to implement any sort of clinical guideline. • Increasing the use of consistent assignment: Shaloom Healthcare Staffing believes The aim is to have the same staff taking care of the same individual each time they work, thus allowing the people to know the residents; know their likes, their dislikes and any early subtle changes when someone is getting ill. They know their routines. When you do the consistent assignment, you’re able to better achieve person-centered care planning and decision-making.” • Increasing person-centered care-planning and decision-making: Radiocw-Healthcare knows it’s really hard to figure out, if you don’t know the routine of the resident[s] you’re caring for, what time they like to wake up, whether they like to bath or shower, what music they like to listen to, what type of foods they like, whether they like to snack or not, whether they like coffee or tea. When you have a consistent assignment, you can actually do much better person-centered planning and decision-making and really tailor the care level to the individual needs, which is what we all want for our family members and ourselves • Safely reducing hospitalizations: Given a lot of these issues about consistent assignment, staff stability, communicating between the nurses and physicians, early detection of subtle changes in individuals, understanding people’s likes and dislikes, particularly around end-of-life counseling — we understand that all of those are critical information for figuring out how to reduce hospitalizations, • Using medications appropriately: We realize that the elderly in general are on numerous medications. Those in nursing homes are on even more medications, and a lot of the medications contribute to many of the problems and poor outcomes.” The Advancing Excellence” campaign initially will focus this goal on reducing the use of antipsychotic in individuals with dementia, because doing so aligns with the CMS dementia collaborative as well as the national strategy for Alzheimer’s disease. • Increasing resident mobility: Getting people up and moving around is critical to their overall health. Adding mobility efforts help prevent falls and pressure ulcers. • Preventing and managing infections safely: This Advancing Excellence goal initially will focus on Clostridium difficult prevention and reduction. The group is partnering with the Centers for Disease Control and Prevention in efforts to promote effective use of antibiotics, since overuse or misuse can lead to illness and hospitalizations. • Reducing the prevalence of pressure ulcers: This goal and the one of decreasing pain symptoms have been the focus of national efforts and the Advancing Excellence campaign for some time. Conversation and Companionship: Studies show that staying social and engaged, increases seniors’ quality of life and prevents depression and cognitive decline. We can provide clients with friendly company, conversation, and mealtime companionship. Meal Preparation: Eating nutritious meals can make health issues easier to manage, help prevent future health problems, help increase energy levels, and keep weight in check. for seniors who have trouble with meal preparation; we can cook hot, delicious, wholesome meals, allowing clients to help with preparation according to their ability. Preparing food together can make mealtime a more social and enjoyable experience. Laundry: Clean laundry is a must, but for some seniors this chore can be too physically demanding to do alone. We can take care of all laundry needs, either in the client’s home or at a nearby Laundromat. Services include washing, drying, ironing, and putting laundry away. We work with clients to make sure laundry is done according to their preferences. Light Housekeeping: Housekeeping help can prolong a senior’s ability to live independently at home. We offer light housekeeping services to include vacuuming, dusting, sweeping and mopping floors, cleaning bathrooms (sinks, tubs, showers, and toilets), cleaning kitchens (sinks, appliances, counters, taking out trash), straightening all rooms, organizing closets and drawers, and cleaning any interior windows that can be reached without a ladder. Shaloom Healthcare Staffing does not provide outdoor cleaning or maintenance. Grocery Shopping/Errands: Illness, weakness, and an inability to drive are just a few factors keeping seniors from working through an ordinary to-do list. Having help with shopping and errands keeps seniors engaged and improves quality of life. We can shop for groceries at the clients’ preferred stores and run errands, such as picking up prescriptions or stopping at the post office. Clients may choose to stay home or come along for the ride as a way to stay active and engaged. Incidental Transportation: One way to help prevent the social isolation, loneliness, and depression common in seniors is to simply get out and about. If driving isn’t an option, we provide transportation incidental to other care services, such as taking clients to medical appointments, barbershops, beauty salons, shopping, meeting a friend for lunch, or wherever else a client wants to go. Being out and about benefits a senior’s mind, body, and spirit and is an integral part of Shaloom Healthcare Staffing ’ Interactive Caregiving™. Medication Reminders: Although Shaloom Healthcare Staffing ensure that clients take their medication as prescribed. We can assist with opening medication containers, reading labels, and reminding the client when it is time for the next dose. Grooming Guidance: Paying special attention to self-grooming can help a senior feel and look better. As a person ages, grooming can sometimes present physical challenges. We assist clients with shampooing, shaving, hair combing and styling, and other grooming needs so seniors can look and feel their best.
