Ardnas Home Health Care has an immediate need of Registered Nurses ~Requirements Current and valid Florida RN License Current CPR card 1 years of experience working as a Homecare Nurse with a Certified Home Health Agency Preferred Valid Driver's License, access to a car and the ability to drive for visits Good working knowledge of OASIS paperwork, 485s, and SOC's Excellent verbal and written communication skills Conscientiousness when it comes to paperwork submission ~Responsibilities Discusses and develops a comprehensive Plan of Care in collaboration with the physician, and by assessing and identifying the clinical and financial needs of the patient/family/caregivers and establishing mutually agreed upon goals. Obtains verbal and written orders from the physician. Provides case management including coordination, authorization and tracking of patient care via the plan of care. Monitors and documents patient's condition and effectiveness and timeliness of care and/or interventions; evaluates updates and revises plan of care to facilitate achievement of planned and expected outcomes.
