Job Responsibilities As a Home Care Coordinator you will be responsible for creating individual nursing care plans that reflect the client's current needs as well as implementing preventative and rehabilitative non-medical nursing procedures as needed. Additional responsibilities: - Assessing client conditions and modifying treatment plan to meet client's needs or changes in condition. - Maintaining accurate and detailed reports according to policies and procedures - Transcribing physician orders, implementing, evaluating client outcome and informing of significant changes in client's condition. - Practicing infection control and safety measures - Educating and instructing the patient, family or staff - Complying with the Agency policies and procedures - Maintaining client confidentiality - Participating in personal and professional growth and development of Personal Care Assistants Job Requirements - Graduate of an accredited school of nursing - Current registered nurse license with the Virginia State Board of Nursing - Experience in Home Care and/or Case Management required - Knowledge of home care regulations and discharge planning preferred - Current CPR certificate - Excellent communication skills - Ability to work independently in a fast paced environment - Good people management and conflict resolution skills Position Type: Per Diem, Part Time RN Salary: Competitive Serious and qualified candidates should send their resume to: [email protected] Location: Chesapeake, VA Compensation: We offer a competitive salary.
