Nurse / Wellness Director Compensation: To be based on experience SHARP, EXPERIENCED, KIND-HEARTED, NURSE / WELLNESS DIRECTOR for MEMORY CARE. Our community culture is based on “Resident First Decision Making.” This requires a passion for service to the elderly. FULL TIME POSITION 29-bed assisted living community specializing in memory care in South San Diego County is looking for an LVN, LPN, or RN with a minimum of a year of experience working with elderly residents with dementia. New grads will not be considered. As Wellness Director, you will be responsible for overall health and wellbeing of residents, including using nursing skills, judgment and facility protocols either directly or through delegation to others for preventive health services, including but not limited to, disease prevention or management, nutrition, skincare, hygiene, transferring, turning and repositioning, activities of daily living and infection prevention. You will provide direct nursing care in emergencies as well as in the presence of illness or disability in order to maintain life, reduce stress and provide comfort. You will monitor the inventory of medications, medical supplies and equipment to ensure an adequate supply is on hand to meet residents' needs. You will practice safety and exposure control measures consistently and effectively. Other duties: * Assess potential new residents. * Assist in admission, transfer or discharge process. * Develop and implement resident care plan. * Administer medications or treatments as prescribed, including applying creams and lotions, changing dressings/bandages, massaging and aiding in range of motion exercises. * Document all resident information accurately on all required forms, charts etc. in keeping with professional standards. Receive and transcribe physician or other health care professional orders and initiate actions promptly. * Attend patient visits by physicians; monitor visits by hospice and therapy professionals, documenting clinical records appropriately. * Notify physicians of resident condition changes, need for additional orders, or to report information that will promote the health and wellbeing of the resident. * Communicate effectively with staff at shift change by sharing important resident care and activity information to promote continuity of resident care. * Supervise and direct wellness staff. Monitor performance of staff for completion of tasks and assignments. * Provide the necessary instruction or correction to staff to ensure the safety of residents, visitors and staff. Leadership skills are a must! * Ensure a clean and safe work environment is maintained, including nursing station, med cart, common areas, patient rooms and hallways etc. * Monitor staffing requirements and report concerns regarding the quantity or quality of individuals on duty. * Notify resident family/responsible party of condition changes in accordance with facility policy. * Promote adherence to all resident rights and address any concerns or grievances expressed as per facility policy. * Must possess a current CA driver’s license, good driving record & current auto insurance. Requirements * Graduate of nursing school * Current California RN, LVN or LPN license * Current CPR certification * 1 years of experience in memory care or elderly setting * Supervisory / leadership experience * Computer skills * Flexible with ability to multi-task * Organized and detail oriented * Excellent English skills: reading, writing, & speaking • OK for recruiters to contact this job post.
