Hello my name is Jessica. I'm an elder care provider, independent/private duty registered nurse, with a total of 13 years of community health experience; 5 of 13 in a licensed nursing role. Recommendations on request. Looking for patients and families that need quality care in their own home, with the same professional nursing treatments you may receive in a facility, or clinic. Each client will have their own personalized care plan, with an in-home medical chart to track, and monitor effectiveness of care. This chart also serves as a way to organize all of the client's treatments and/or medications, and aides in communication with all of the client's providers, and caregivers. Services are provided within the scope of the Hawaii Nurse Practice Act. As of now services are private pay only, although this may change in the future for certain services. Hope to hear from you!
