Elder Care Job in Valley Stream, New York

I am looking for in home health care for my brother. He is almost 72 years old and gas stage 1 diabetes. He is prone to infections on his legs and has bee hospitalized 2 times and the last one resulted in amputation of his little toe on his right foot. He has stage 4 kidney disease and we are preparing for dialysis. He lives in an apt. In a senior citizen complex in Valley Stream, N.Y. I am his sole caretaker. I do his shopping, pay his bills and take him to his doctor appointments. He is totally dependent upon me. He is reaching the point where he is not capable of taking care of himself and I cannot do more than I am presently doing. I work full time. I check his legs weekly and treat them if I see the beginnings of sores. Last week I went to treat his legs and saw that the bandages I had put on his leg had come loose and a large sore was exposed - for how long I do not know. He can't comprehend when something does not seem right. He needs someone to check on him daily. I can't do that. I need advice on where to turn to get some help for my brother.
