• Bath and dress residents, patients or clients allowing them choices and opportunity to complete task as tolerated • Assist or serve meals • Take, record and report vital signs, input and output and weight as required • Reposition or turn resident, patient or client as note in care plan • Communicate to health care team changes as observed • Empty bed pans and provide assistance when requested with bathroom services • Respond to resident or patient call lights • Answer resident, patient or client calls and record message as needed. • Observe for bruises, bedsores, blood, urine or other injuries or wounds and report to charge nurse • Provide a clean and sanitized living environment by completing housekeeping duties as required • Change bed linens and ensure rooms are restock with supplies and fresh water • Transport resident or patient to other treatment units. • Provide comfort and conversation to resident, patient, client, family or caregiver • Proven efficiency to engage individuals forming immediate connection resulting in understanding needs and expectations.
