Description of Skills: Services Include: Customized care plans bases on the individual, medication administration and management, assistance with personal care/maintenance and activities of daily living(bathing, dressing, grooming, toileting, feeding, etc.), meal planning, prep, and assistance with maintaining special diets and restrictions, cleaning and household management services, provides physical assistance and mobility(lifting and tranfers, repositioning, etc.), provides restorative and rehabilitative care, provides transportation to appt. and errands, provide companion and social/mental support and activities. Have extensive knowledge and experience ostomy and colostomy care, peg tubes and feeding machines, wound care, dementia/memory care, post operation and rehabilitative care.
