Our mission is to maximize individual independence, dignity, and quality of life for adults, as well as for their caregivers, by providing exceptional adult day health care, advocacy, and education. WHY ARE WE HERE? • To help a family member who has been diagnosed with memory problems, dementia or Alzheimer's disease, or is physically or intellectually impaired. • To help an adult with a debilitating disease who requires feeding, socialization, or medical supervision. • To help an elderly individual who is not capable of or limited in his/her ability to fully live independently, but does not require 24-hour nursing care. Daily Living Center allows family and friends to attend to their daily lives knowing that their loved one is being taken care of in a positive, safe, caring environment. WHAT SERVICES DOES THE DAILY LIVING CENTER PROVIDE? • A safe and secure environment. • Socialization and Recreational Activities • Health monitoring and person-centered care by licensed professionals. • Nutritious hot meals and two snacks. • Bathing for an additional cost. • Hair Salon by appointment only. THE ADULT DAY HEALTH CARE CENTER: • Is open 5 days a week, 7:30 am to 5:30 pm. • Accepts adults 18 years and older without regard to race, religion or natural origin. • Can provide wheelchair accessible transportation. • Have a Caregivers Support Group meeting on the third Monday of every month. • Accepts Private pay, Medicaid, Long Term Care Insurance and is Veterans Administration certified. • Current cost is a reasonable $80.00 a day A non-profit 501 (3) corporatition A United Way Agency
