Medstats Home Health Agency location serving Northern Virginia that supports Elderly, disabled and medically ill adults with daily living activities and care at the person's home . We are looking for a PRN/PART TIME Registered Nurses who can complete assessment and Reassessment and train caregivers. We are also looking for companions who are CPR and First Aide certified and transportation!!!! Key Responsibilities for COMPANIONS :(Transportation with insured/ CPR First Aide) • Provide companionship, friendship and emotional support. • Talk, listen, share experiences, play games/cards, read to client etc. • Help keep clients in contact with family, friends and the outside world. • Provide transportation to medical appointments, grocery store and errands. • Accompany clients to recreational and/or social events. • Assist with plans for visits and outings. • Write or type letters/correspondence. • Organize and read mail. • Plan trips and outings and possibly travel with clients. • Teach/perform meal planning and preparation. • Perform light housekeeping. • Participate on the Care Team by providing input and making suggestions. • Ensure service is delivered in accordance with Agency policies, procedures and industry standards. • Monitor supplies and resources. • Evaluate the program and make recommendations, as indicated. • Follow the written care plan. Key Responsibilities for Registered Nurse: Perform the initial evaluation visit and regularly reevaluates the patient's nursing needs Initiates the plan of care and necessary revisions Prepare clinical and progress notes for each patient visit Counsel the patient and family/significant others in meeting nursing and related needs Process orders and notify physician of patient needs and changes in condition Train caregivers according to plan of care Assures proper maintenance of clinical records in compliance with local, state and federal laws
