Elder Care in Melbourne, Florida

I am a Florida RN and a certified case manager. More often than not, I see our elderly loved ones released from the hospital or a skilled facility without the proper education or follow up. Some are released too soon and that can cause any number of problems. My goal, is to see as many of our seniors cared for in a way that prevents negative outcomes such as a readmission or medication confusion etc. I currently work full time, so I am starting out with this new venture in the evenings and on weekends.
