I am a Certified Nurse Assistant and a Bachelor's Degree Social Worker, with experience in family care giving as well as Nursing Home Experience. As a social worker, I have connections within the community to help and assist, if needed, with resources and connections. This is reassurance that your loved one will be provided the best care and can be directed to resources the family may need at any given notice. I enjoy and have a great compassion for working with the geriatric population and have found it fulfilling to provide my services to clients, in their time of need. I believe each person deserves love and compassionate care from someone who is willing to walk with them through every phase of their care. Below is a description of services I offer. Pay is negotiable and mileage is included in the rate. • Assessment of needs • Provide assistance with Activities of Daily Living (ADL's) o Bathing o Transferring (walking) o Eating o Continence/Incontinence o Dressing o Toileting • Transportation to and from doctor appointments and errand runs (pharmacy, grocery, etc. mileage is included in the pay) • Daily Vitals check (morning and before end of day) • Medication reminders • Nutritional meal preparation or planning • Experience in working with individuals with Dementia and Alzheimer's • Local Resources in Community (Meals on Wheels, etc.) • Educational Materials • Light housekeeping (all materials will be supplied) • Companionship
