Delivers direct patient care; assesses patients physical, psychosocial and/or emotional needs; provides patient and family teachings; Attend to medical needs of patient which includes taking vital signs, blood sugar, giving medicines and assisting patient to his activities of daily living etc. Administers medications as prescribed; serves as a resource person for knowledge of medication uses, side effects, and dosage; checks for proper medication order; documents medications taken; assist patient on activities of daily living such as taking a bath, preparing nutritious meals, eating appropriate meals based on patient’s condition, doing daily exercise, providing clean and proper room environment by doing light housekeeping; Communicates with patient’s needs and makes referral; Assisting patient in further medical intervention such as doctor’s check up and laboratory needs; Around the clock service; Maintained personal hygiene of clients and monitored health.
