I am Claudina, a Registered Nurse in Singapore and Philippines. I am seeking a caregiver job. I have 10 years experience in patient care management, risk management and patient education. I have attended trainings pertaining to elderly care and management such as Intermediate Care Area Nursing Course, Diabetes Management, Wound Care Management, Rehabilitation in Elderly and a lot more. � Provide the coordination and care management for the assigned group of patients. � Implement and evaluate nursing care activities for patients and direct operational work. � Participate in clinical ward rounds with doctors and Nurse Clinician and reviews daily action and treatment plans of patients/patient related activities. � Discuss with Nurse Clinician or doctors on any change in patient�s plan of care/updates /related activities. � Perform a variety of simple invasive and non-invasive procedures related to care plans, within the scope of duties. � Assist doctors during clinical procedures, examination and clinical assessment of patients as appropriate. � Assess patient and develop discharge plan based on patient/family needs. � Implement discharge plan and intervention as appropriate. � Maintain objective documentation on the assessment and evaluation of patient�s condition. � Perform accurate data entry in patient acuity system. � Participate in the development and implementation of nursing care activities. � Perform basic nursing care activities independently. � Evaluate nursing care by monitoring and perform concise recording of observations and vital signs. � Assist doctor and nursing staff during treatments and examination of patients. � Institute and maintain safe precautionary measures. � Perform designated procedures such as venepuncture, blood glucose monitoring and 12 lead Electro-cardiogram. � Maintain objective documentation on assessment of vital signs and reports change of patient�s condition to the Registered Nurse promptly.
